RFE received on Birth Certificate

In response to my Birth Certificate RFE (since the certificate was issued 27 years after the fact – I guess lot of us are getting the same REF), I planned to submit notarized affidavit from my parents. I would appreciate if someone could post the format of the affidavit (or what exactly goes in that affidavit) letter to help me out here.

Case details:

ND: 11/20/01
2ND FP: 6/15/03
3rd EAD Applied: 7/01/03
AD: ???
 
I was in a similar situation as you. I got approval within two weeks of sending the RFE.

Please see the attached doc for sample affidavit of birth that I used :)

Here you go. Sorry, I didn't know that attachment had to be in pdf format.

Please be sure to obtain the affidavits from two persons, parents, uncles, aunts etc; someone who can confirm your date and place of birth.
