I finally receieved my RFE (Request for Evidence) notice :) By the way, my RD is May 1, 2000, so it looks like they're moving faster. Anyway, they want 3 things:

1. Completed Biographic Information Sheet - Form G-325A
2. Medical Examination form - Form I-693
3. Supplemental From to I-693 (Documentation for Immunization)

Here are my questions:

1. They want all the evidence to be returned together in an envelope they provided, yet they want #2 and #3 to be filled out by a "civil surgeon" in a sealed envelope. Does the civil surgeon fill out the forms, put them in a sealed envelope, then hand it off to me to send out together with G-325A (which I have to fill out myself)?

2. Do I provide the civil surgeon with the blank forms in #2 and #3, or do they already have them?

3. Does anyone know a "civil surgeon" that I can go to in central NJ?

4. What exactly does the civil surgeon do in terms of a medical exam? Is there a blood test? What do they test for? Anyone been through it and want to share their experience?

5. How do they know what immunizations I already had? I obviously wasn't in this country when I was immunized! Do they just take my word for it, or do I have to provide them with evidence from home (which could take some time!)?

You will get a sealed envelope from the doctor's office, then enclose it in your envelope along with item #1 and mail it to INS. You have to do it, naturally doctor's office is not gonna do it for you.

They have the forms.

You have to pull the list of "INS-authorized" doctore by state and pick one - you can't just do it anywhere. It has to be from INS list. Follow the link: http://uscis.gov/graphics/exec/cs/Index.asp?

4. What exactly does the civil surgeon do in terms of a medical exam? Is there a blood test? What do they test for? Anyone been through it and want to share their experience?

Here is what I did (I provided medical with my original application). I went to doctor from INS list, they gave me the list of immunizations needed for the paperwork, specifying which ones MUST be done at their office and which ones can be done somewhere else (ask that question). I then took this list to my primary physician under my insurance health plan, and asked for tests that are covered by my insurance. After getting the test results from my insurance I make trip #2 to the INS doctor. They do all the remaining tests and schedule next apptm. Just do whatever they ask you to and they will tell you when to come for the final envelope. I spent around $80 for this package and saved some money on tests covered by my insurance. If you do everything at INS doctor's office, it will cost more (of course it depends on their specific pricing too, it is different at different places).

5. How do they know what immunizations I already had? I obviously wasn't in this country when I was immunized! Do they just take my word for it, or do I have to provide them with evidence from home (which could take some time!)?

Again, this depends on particular doctor. They will ask you which ones you had, They might take your word for it or make you repeat it because there is no evidence. My advice is just do everything that is needed and be done with it.

Hey Punky, I have a q for you: did you travel on RTD after your asylum approval and now? I am trying to see if there is a trend for INS asking all RTDs and travel information - whether it depends on your situation or not.

Yes, I travelled once on RTD after my asylum approval.
 
Punky, did u send them a copies of visas in your RTD and I 94's with your initial application?

Nope, I didn't. Is it unusual that they didn't ask for that? Looking back at my records, I had 2 RTD on which I travelled one time each. One was in the 1 year between getting asylum granted and applying for I-485 (1999), and the second right after I got the receipt notice for my I-485 application (2000).

They can figure out what shots you already received by analyzing your blood sample. That was how they figured mine.
