My wife got an RFE in her 485 application. Our marriage certificate has her maiden name. An affidavit that we made in India has her maiden name too. We decided not to change as we didn’t want to run in to problems when we went to stamp her H4 visa. We had only 2 weeks and there was no time to change her name in the passport before we came to US.
Since coming here (over 4 years) she has been using her married name (Drivers License, Social Security Card, New Passport, School, employment records, etc.) We have to create a statement (affidavit) saying that she has taken her married name permanently. I as wondering if anybody have run in to similar problems? Do you have a statement format that you could share? Please Help !!!
