RFE for I-683 suppliment form

Does any one know or anyone was issued RFE for missing I-683 medical form ? Does anyone know what exactly is that ? I cound't find the form on INS offical web site. Need guidance.

My wife got the same RFE

It actually means that the INS approved doctor forgot to include the Form I-693 in the sealed packet. which is ridiculous.

As my wife did not take the TB test it could mean that too. So best thing is to contact the same doctor and ask them to go through your file to see if something was missing and resend the package asap.

Thanks for the reply. Ya, you are right , this is ridiculous and unacceptable. I will check with the doctor. But did you reply back to your wife's RFE ? Did the doctor just had to enclose the document or did she took TB test and other vaccinations (such as polio, hepititis B etc... listed in the RFE notice ?)

supplement form

This form just contains list of vaccinations u took and if any were pomitted, for what reason. It will say like this vaccine(s) ar enot provided because of 'age criteria'; these are not provided since there is not enough time etc...
hope this helps when u talke with ur dr
i got a copy of this from my dr. this is how i know abt it.
 
RFE I693 supplement form

Me and my wife got the same RFE , we need to go to the doctor again and ask him to fill this I693 supplement form. This is a form that states that you had vaccination that prevents diseases like mumps, measles, rubella, polio, tetanus diphtheria toxoids, etc.
I heard that this form was not a part when my medical was done in Sept 2001, hope this helps
 
I got the damn RFE

for my medical being incomplete.

The incompetent doctor's office forgot to put a simple check mark against the column in the I-693 supplemental form stating that I am up to date on all of the vaccinations. On July 9th an RFE was issued and had to go back to the doctor's office with the RFE letter and the medical report that was sent back by INS.
They referred their records and said that they did not have any info regarding my MMR shots. So had to go to my PPO who administered them and got the copy from their office and gave it to the Immigration Doctor's office.
I did not even get an apology nor did they show any remorse. When I asked as to why this happened, all she said was technically, this incomplete report should not have gone out but they can't help it.
After all this, we found that INS had put the wrong form number on the RFE notice (instead of stating RFE issued for I-485, it said RFE issued for I-765). This typo was overlooked by us as well as our attorney and the result was that this response package went to the wrong department and then became untraceable till the eve of the deadline of October 4th as mentioned in the RFE. The lawyer had to ask for a timeline increase for a fresh medical report to be submitted. Thank GOD, the lost package was found and our application processed eventually and we got the approval stamped y'day.
We are seriously thinking of levelling charges against the INS Doctor's office for negligence which led to tremendous amount of emotional stress and mental trauma and also the unnecessary payment of 460 USD that I had to shell out for EAD and AP extensions.
Any suggestions?

Have you tried asking the doctor's office for the forms ?

this is what the INS website days ...

"Note that the vaccination supplement accompanying Form I-693 is not included on this internet posting. The Centers for Disease Control and Prevention (CDC) have sent the vaccination supplement directly to the civil surgeons for them to photocopy, fill out on behalf of each applicant, and attach to Form I-693 for submission to INS. "


Or next bet would be the lawyer. They should also have it .
 
Ya I'will check with the doctor

I have been trying to talk to the staff at the clinic. they are not
helpful. Planning to go personally. I don't expect that my lawyer would have the form. Per your msg. doctor is supposed to have it. I shall insist on the doctor.

I made a visit to the doctor's office, the staff there checked the records and found the original I-693 suppliment form, apolozised and simply enclosed with the forms that INS returned to me and sealed the envelopes. But question is:

1. Is the doctor required to re-do the paper work ? Because that I-693 form was revised on 4/02.

2. The RFE notice stated that certain vaccination including Heptitis B and influenza type B are required. But I didn't get vaccinated for these 2. Will it be ok.

I want to ask the EXPERTS here instead of my lawyer. I would mail the envelopes back to lawyer upon seeking your suggestion.
