revalidation of H1 Visa

Hello:

I need to some info regd the revalidation of H1B visa

1) what forms & Procedure to be followed
2) Application fees ( I heard it is like $100 + $75, Pls let me know)
3) Application fees Payable to whom ?

Any help in this is regard is highly appreciated.

But I have another question, Application fees and reciprocatory fees is Payable to whom? is it "US Dept. of State" or to
"INS" or "BCIS", becas on the home page of this site immigration.com, it says that


"The INS will cease to exist as of March 1, therefore filing fee checks for all applications must be made out to either the INS or
the Bureau of Citizenship and Immigration Services (BCIS). For answers to other frequently asked questions, please click here. "
I need some clarification in this regards
