Hi,

I need to send mine (H1), my wife's (H4) and my daughter's (H4) passports for revalidation. I am little confused about my family's documentation.

Do I need to send a separate DS-156 for my wife and daughter?

What is the fees I will be paying in this case (the website says, "a reciprocal visa issuance fee in the H, L, O, and P categories of $50 per applicant. Reciprocity fees are in addition to the $100 nonimmigrant visa application processing fee that each applicant must pay"). So I will have to pay a total of $150+$300?

I have gone through the website and I still have these doubts.

Every applicant including minor children needs a separate filled out DS-156. Adult males also need DS-157.
The fee you need to pay is (application fee + visa issuance fee) X number of applicants. In your case it will be $150 per applicant for 3 of you, total of $450.
