RESIDENT PHYSICIANS ON H-1 B--FUTURE?-JOB Vs FELLOWSHIP

I am an internal medicine resident on a h-1 visa. I dont know if it is better to go for a job and secure your green card first and then do a fellowship or to go for a fellowship at the earliest as your chances are best while in residency. I am sure there are a lot of us in similar dilemma and it would be great to excahnge useful info. the options available are: 1]Job first, then green card, then fellowship.
2]fellowship first on a h1, then O visa, then green card[you have to be really good]
3]fellowship first, leave country for 1 year and come back.

I have a friend who is studying MBBS and doing his house surgeon. He would like to come to US and do MD here , which is the best way to get him here. What are all the risk involved?
