Hello friends,


hope some one has answers to my below questions


What documents are required for filing i485

do we need W2 forms? if so for past how many years do we need to submit?

My lawyer asked me to submit the following documents whatever is applicable to me.

HOPE THIS HELPS.

In addition to the applications for Adjustment of Status, you and each of your dependent family members must provide the following:

(1) Two photographs of the applicant for each application being filed (i.e. - I-485, I-765, I-131. The G-325A and IRS 9003 are not considered applications. They are merely supporting documents.) The photos should be taken separately with a white background, glossy, unretouched, and not mounted; dimension of facial image should be about one inch from your chin to the top of your hair. You should be shown in the photos in a three-fourths frontal view showing the right side of your face with the right ear and both eyes visible. Please do not wear any jewelry or glasses. Your name should be printed lightly with pencil on the back of the photographs. We will provide an example that shows exactly how your photos should appear, but because the photographs should be taken within 30 days of your application date, we will forward them when we provide the forms for you to complete.

(2) A medical examination will be required by the INS for each of you. Our office will provide medical examination forms and a list of authorized doctors that can perform your medical exam in your area. One thing that you can do now to expedite this procedure is obtain vaccination histories for yourself and any dependent family members now since they will prove helpful at the time of your examinations.

(3) A notarized copy of each of your birth certificates. Please be sure to carefully read the following information regarding the validity of your birth certificate for immigration purposes.

A notarized copy of the birth certificate(s) for yourself (and spouse) containing your full name, date and place of birth, as well as your parents' full names. If your birth certificate is not available or was not previously registered with the proper civil authorities AT THE TIME OF YOUR BIRTH, you will need to submit separate affidavits prepared by two (2) family members that have personal knowledge of your birth. The INS will not accept birth certificates registered years after your birth. The affidavits must be executed before an official authorized to take oaths (i.e., a magistrate, commissioner of oaths, justice of the peace or the like) and should include the following:

(a) that the applicant's birth was not registered;
(b) the full maiden name of the mother of the applicant;
(c) the full name of the father of the applicant;
(d) the date of the applicant's birth;
(e) the place of the applicant's birth.

(4) Two notarized copies of your marriage certificate. If either you or your wife were married previously, you’ll need to provide evidence that the previous marriage was legally terminated.

(5) Notarized copies of your educational credentials - your university degrees/transcripts or a credentials evaluation report (if applicable).

(6) A notarized copy of each of your passports. This should include all biographic information, visa pages, and arrival/departure stamps. Please make certain the copies are clear & legible.

(7) A notarized copy of the front and back of each of your white I-94 cards. Again, please make certain the copies are clear & legible.
(8) Notarized copies of all Notices of Approval since your first arrival in the U.S. This should include all periods of your stay in the U.S. even if you were in a visa category other than H-1B or H-4. (This should be provided for all applicants.)

PLEASE NOTE: Translations are required for any document not in English. Additionally, there must be a notarized attestation attached stating that the translator is fluent in both languages.
