Required Documents for EB1-EA

As for me, the minimal required documents for EB1-EA self petition are
Fees $135 and I-140 Form. I am also planning to submit following docs:

Current H1-B Visa
Passport, former F1 Visa and I-94
Spouse\'s Passport, F1 Visa and I-94

and all the supporting documents: index, exhibits etc.

Is there anything missing? Just want to make sure I didn\'t miss anything.

Should be OK

I think all that you have is fine.

You need:

1. You I-140 application and credentials (transcripts, degree certs),
   CV, publications, merit certificates, membership, papers,
   research reports, brochures from your companies etc and stuff
   media about you.
 
   Pay stubs, employment verification, recommendation letters.

2. Immigration Docs: All relevant immigration docs about your previous immigration status etc.

I-140 question

I believe Pay stubs, employment verification is not necessary for EB1-EA.
I just received my H1 approval notice. When I fill I-140 I-94 #, should I use the # with H1 approval notice, or the # issued when I entered US half a year ago.
