Request for initial evidence

i recieved this letter today for RFE... i never sent medical so am planning to send that. It also ask for G325A completed and with ORIGINAL signature. - what does this mean? i already sent my G325A with initial application. should i sent another one or are these standard stuff written in every RFE form? i remember signing my whole name in the G325A forms instead of my usual scribble signature.... could this be the issue? should i send another one with my usual scribble of signature? thanks.

Same thing happened to me. Fill up the G325a form and send it again. Do exactly what the RFE said eventhough you had sent it earlier.
