Replacement EAD(Jaxen, Jim help plz)

I am applying for replacement EADs for me and my spouse. We never received the EAD cards the first time. 0ne of the supporting documents specified as needed is copy of EAD , as i dont have the EAD what do i submit?

I really appreciate your advice.
