Reissue of OCI on new passport issue

We have OCI's for our kids since they were a year old. Now that they are 5, their US passport expired and I got new ones for them. Per the OCI website, it is best to get the COI transferred or reissued to the new passport through what they call Misc services. But their online application is such a pain- no clear instructions at all. It does not clearly indicate where you are supposed to put in your old passport # and where the new one. So when I filled it out what made sense to me and got the print out, it shows the old passport# with the new issue date.
In short, my question is - does anyone have a sample filled out application form or a link to an example which has instructions next to the blank for eg- "put old passport # here" etc

I really dont know why they make it so difficult!!
Any help would be appreciated

------ you can again take the printout. Now Try to enter info again. write the new passport info at other passport number. Trail and Error . Once you get the Form Printed with correct Information then let us know what is the correct way to take printout with correct information

----------I don't know about any"Sample posted". who got the OCI sticker in the child new passport he/she can help you.
I really dont know why they make it so difficult!!
