I have few questions regarding recommendation letter

1) whom should it be addressed
2) is it needed to put date
3) do i need to ask people to send reco on letter head and mail me or signed pdf print will work?
4) how long this letter is needed to be
5) do i have to attach CV of the person writing or just summary of their work mentioned in one para in the letter will do?
 
1. Its upto you. What I did was "Dear Immigration Officer" with the full USCIS address before that.
2. Yes. The date shows whether the letters are not old enough.
3. For effectiveness: Yes. The format is upto them and upto you. What you want to do is collect all signed letters and manage them the way you want. There is no seal of the referee required.
4. 2-3 page is enough. Shorter the better.
5. This is not a mandatory thing but its better to request CVs of your referee. You can make their CV yourself if you know them better (but its easy to request them). The better their CV, the better your case appears.

Some of my letters were addressed to "to whom it may concern", others to "Dear Sir/Madam" and "Dear Immigration Officer".

Yes

It is better. Try to attach as many letters with letter heads as possible. If they just send you email in PDF format, print it using colored printer.

4) how long this letter is needed to be

2-3 pages and should clearly describe your conntributions and discoveries. Each letter can describe one different point and how your work is important to nation, and try not to make them redundunt. Try to get letters from people from different states; at least from three different states as you need to prove that your work in important to whole nation.

For NIW, not that important but they have to introduce themselves in the first paragraph in the letter. You can get their CVs online and print them out if you want. I never attached any CV.
 
You should absolutely try to get your letters on letterhead. This is more important if the letters are from government agencies. If they are, CV is not very important, as the position of the person writing the letter conveys its importance.
