Received only passport without Approval or Rejection

My nephew received a 221g when he went for interview back in December. He furnished all the required documentation as mentioned in the 221 g document and as instructed in 221g, he went to the drop box office in Hyderabad and gave all the documents around Feb 1st. Today (March 8th), he received only the passport back from the Consulate. There is no approval stamping or anything to say that it is rejected in the passport. He also did not receive any other documents from Consulate. All the documents that he submitted are still at the Consulate.

Does anybody know what could be happening? Is his visa approved or still pending? Would the Consulate be sending a separate letter? What is he supposed do now? Is there any way we can check with Consulate?
