My parents got their GC last March (2008), this March (2009) it will be one year, but they were abroad the whole time. Now due to a hip surgery, my father is not able to walk for a couple of months.

It will be a while before he can do any kind of travel. How can we get the SB-1 (return resident visa). Is there any other letter i need to send to USCIS mentioning that they are not able to come due to medical reasons.

What kind of letters do we need from the hospital. Should he go before 1 year expires to the consulate abroad, or can he go later too.

The consulate may legitimately question why they were abroad prior to the surgery. The SB-1 is hard to get. If your father had the surgery right after getting the GC, he might have a case.
 
Got GC in March, had operation in November. Don't know if that still helps. What is the other scenario?

Give up GC and go back to Visitor Visa?

It doesn't help, since he had eight months to return to the US and chose not to. The SB-1 is generally for cases of involuntary stays in a foreign country, for health reasons, incarceration, military service, etc.

Probably, since they don't seem to want to reside in the US.

there may be a reason...like medical fees is the main reason they decided to go back and look around then the actually surgery starts on Nov. maybe still a chance to get SB-1.
