RECEIVED DENIAL LETTER - help explain please

I didn't forget two trips, I just had two trips, AFTER I submitted my
application, which I told her about.
I did, however, write a note on my application, that I made several trips a year across the border that I had no record of.
My husband didn't have a problem when he filed and neither did I.
We both wrote down the same statement.

Did you voluntarily tell the IO about the two trips that you had originaly forgotten to mention OR he/she told you that you had forgotten to tell the IO about those two trips specifically, so then you had to own up (so to speak) to the two trips?

Also, in another post you mention that your FP letter was lost. How did you find out it was lost? Mine may be lost but I cannot figure out if it is lost...
