Receive packet 3 what to send? URGENT

Mine is a typical CP-Mumbai(Bombay) case and I am working for our company's Mumbai office.

I have just received packet 3. It says that I have to just fill and send DS-230-Part1 for me and my wife along with the Bar code sheet.
#1Does than mean that I must ONLY send:
1)DS-230-Part1 of self
2)DS-23-0-Part1 of my wife and
3)Bar-Code

Than I can get get the PCC, BC, Marraige cert. etc till i get the interview date.

#2Also, in the form, they have said that I can attach additional sheets for the work (i.e . Col 22-List below all employment for the last ten years). in that its just the employer, location, job title and from/to. so I guess nothing else is required (i.e. no job description ,etc)...is it right?

#3Also, suppose my employer is the same i.e. headquarters is in US and branch in India . So do I need to make two entries in Col. 22 (if I have worked for 1 year in US and now for the same companies branch office in Mumbai for the past 1 year?

{Once you send out the Packet 3 you would have roughly
2 1/2 to 3 months for getting all these documents ready}
