Reasons for RFE during I-485

I see that quite a few people have been getting RFEs during their I-485 process. Could you please post your RFE experiences, especially with regard to the following:
1. What was the reason that INS issued the RFE. Was it totally random or were any documents not included while you filed the I-485?
2. What kind of documents did you have to provide INS in response to the RFE?
3. Did the RFE notice go to the attorney to the primary applicant?
 
One possible answer for RFE

My friend informed that he got RFE during 485 process. The reason what he mentioned is when he fillup the forms during 485, he didn\'t sign one of the application which was not given by lawyer during that time by mistake. Later when he got RFE, he signed the application and also submitted his later paystuff. Then everything is smooth. He got his GC last month.
 
Did you get RFE for employment letter or tax returns?

If you got an RFE for an employment letter or tax returns, could you please post your experience? Also, did the RFE go to the attorney or to you?
 
Other reasons could be Medical/Photos etc

If the doc screws up while completing the medical forms, you could get an RFE.

Similarly for the pics.
 
Here the question is again,

Is the RFE regd employment letter and paystubbs etc. is due to what?

a. Changing attorney by the company in the middle of one\'s GC process?
b. Changing job buy one, after filing I-485
c. Any of these?
d. None.

Please share RFE experiences for the recent RFEs (for people from June 2001 filings). Thank you in advance.

RFE was for X-ray for wife. She was pregnant during that time.
RFE was received by lawyer.
RFE received by INS (VSC) on 03/12/02 and case approved on 03/18/02

My first name and last name got interchanged

Is there any possibility of getting RFE??

I got rfe for because of the date format used in DD-MM-YYYY

I got RFE for date of birth. My date of birth is August 10th, 1959.
In my passport(India) it is printed as 10-08-1959. So he confused with my date of birth whether it 10-08-199 or 08-10-1959.

My lawyer is going to answer for this.

Did any body got this kind of RFE?
