Hi,

What could be the reasons for someone's I-140 getting denied?

I know 2 of them

If they feel that company is not in a position to pay.
If they feel your educational qualifications do not meet the criteria.
These are the most commong RFE's for I-140 and in case of denial
I think you will have chance to appeal.
