Re-issue of stolen H1B and H4

Guys,

I found this at website. Check this out. The situation is I got stolen my H-1B and H4 for my wife.

According to this, I can get re-issue of my visa at St.Louis office.
In this case, do I need to submit DS156 /157 as I did for my 1st application.
What I mean is, I need to send all the documentation which is needed for 1st visa stamping ? If so, I think I should send photo and fee ($100 per person) and all other stuff. I got police report and copy of original visa and I-94.
I have new passport, too. But I don't have new I-94.

Any advise on this will be appreciated.

But according to their web site, they re-issue visa for lost/damaged/stolen.
I know that renewal and revalidation is no longer in service, but this different case.

Due to the discontinuation of domestic visa revalidation services, all applications for replacement of a damaged, lost, or stolen visa must be received at the processing center in St. Louis no later than September 30, 2004.

I know that I don't need visa as long as I stay in US, but we plan to go on vacation out of US and need visa to re-enter US.

Can't help you (If in doubt send all the supporting doucuments) but curiously what will happen after 9/30?
