Re-entry permit filing requirements for expedite processing

Visitors of this forum kindly let me know if I am missing anything from the checklist:

1) completed I-131
2) supporting documents for expediting the process
3) fees $385 payable to U.S. Department of Homeland Security
4) an official photo identity document showing photo, name and date of birth.
5) two pre-paid, self-addressed express mailers, e-mail addresses, and a fax
number

Also, I remember from somewhere that I-131 can be filled online and supporting docs can be sent separately. Is this true?

I have my permanent address of Redmond, WA, but want to get the biometrics done in San Francisco. Can this be done by stating my SF mailing address in Part 3?

My checklist seems to be correct as I sent application via express mail on 30th April and got notice via express mail on 6th May for biometrics 11th May.

I'm very curious to know what your reasons for 'expediting' your application were and what your supporting documentation was.
I'm going to be teaching abroad and leaving by July 7, 2009.
I need the biometrics done before then, but I am asking that the actual Re-Entry Permit be issued in Japan (where I will be teaching) so I don't have to worry about picking it up here in the US on time.

Any input you can give me on this matter would be VERY appreciated.
