[RFE question
NV2001- in one of ur earlier posts u had mentioned u received a medical RFE - I have just received one which states that my immunization is not complete. I'm thinking my doctor made some mistake in the immunization form. If you don't mind I have a few questions -
-Which lines did ur doctor leave blank? In my case the Hepatitis B line was blank (is hepatitis B vaccination necessary? if not should something be filled in that line?). Also there are 3 lines with MMR on it. (I have taken MMR). He has filled the first MMR line but left the other two blank. Is that right?
-under "Results" on the immunization form - which box did he check when you resubmitted? - was it the one which says "Vaccine history complete for each vaccine, all requirements met"?
-did u include a new I-693 and immunization form when u responded to the RFE or did u just send in only a new immunization form?
-is it ok to go to a different doctor the second time?

You posted that in a thread that was tracking an entirely different issue. So I have started this new thread for you.

In my supplement form to i-693, the staff at the medical center forgot to check the last three boxes. They relate to Verecelli and influenza and I don't know for what f******g reason. That led to another RFE and a lot of delay.
They checked "....incomplete..." because of TD Vaccine shortage at that time.
I had submitted only the supllement. Thats what was asked in RFE. You may go to a different doctor but the old one did this new supplement for free. Hope this helps.

Is Hepatitis B vaccine necessary?

NV2001- Thanks a lot for starting the new thread and answering my questions. Sorry to bug you again but I have a couple of questions more -
- I have taken a fresh immunization report from the doctor to respond to the RFE. I am very concerned that he has left the entire Hepatitis B and two of the MMR lines blank. Is it wrong to leave any of the lines blank? IS HEPATITIS B VACCINE NECESSARY? IF NOT WHAT COLUMN SHOULD BE TICKED ON THIS LINE?
-Td vaccine is now available and I've taken the first dose. Second dose is after 1 month. Do I need to wait and include that or is it ok to submit the forms now?
- were your forms all typed out? Mine is so shabbily hand written that I'm concerned about it.

I have these observations:
If you are below 40, then you must get MMR shot and doc should certify that you did. Secondly I don't believe that there are two doses for TD shot. Its once in 10 years thing.
I am not sure about the Hepatitis thing either. However INS will be satisfied only if the civil surgeon certifies that all immunization requirements are met. This is the most important.
