Hi All,

I plan to get an affidavit from my parents, as my birth certificate was issued just last year.

Is there a prescribed form of "Affidavit" in order to be accepted by INS. If so, please provide complete details..

Doctor list is available at ins website.
If the birth certificate is official you are fine. You don\'t need affidavit. However, affidavit format is available at immihelp.com.

Also, I read some where if the registration date is recent in the birth certificate, then it is good to have an affidavit from parents. Is that true??
