Quick Question with regards to Name change on N-400

I am filing N-400 for my wife (May 2009) will be her 5 years in the US on PR.

We got married last year. I had a quick question regarding the change of name on the N-400 form - she didn't change her last name to my last name after marriage and now she wants to.

Do we need to attach any documents for changing her last name to my last name?

We have our marriage (church) certificate from India. Her passport states that I am her husband. All other documents i.e. PR Card, Driving Licence, shows her preivous last name.

No, you do not. She can legally change her name to whatever she wants to without providing any additional documents. I am doing the same thing when she applies in September (I also got married last year and my wife did not take my name but she will change it once she applies for naturalization).
 
She should bring the marriage certificate to interview along with checking name change option on N-400.
