Quick i-485 Part 3 Question? Help

Need some help filling out I-485, part 3. Any help would be greatly appreciated, in the area where it asks for my Nonimmigrant number and consulate where visa was issued, do i put down my I-94# and Dublin (as this is where i was inspected) or write none, as Part 1 asks for my I-94# also?

Also one more thing, myself and my wife are sending everything (I-130, I-864, I-485, .etc etc) together from TN, so do we send this to the Chicago Lock box facility or a service centre?

I am assuming you entered the US using VWP? In that case, you do not have a non-immigrant visa number since you were not issued one.
