quetion on employment letter

I have some questions about the employment letter needed.

Who should write the letter? My boss or human resource personnel?

What should be specified in the letter? Is it fine just to have 3 setences, saying I am employed, my position and my salary? Or it needs to be more specified and long?

How to get it notarized? Can I do it at a bank?

The letter can be written by your HR.
It should describe your job in detail along with salary offered. The description should match closely to that in the LC. It should also explicitly mention that the position offered is permanent and full time.
It should be signed by HR representative in front of a notary public and notary should then put his seal signature and date.
 
I got ER from my HR. The letter says I will be paid XX,XXX$ per year and currently I am getting paid XX,XXX - 10K. This is the only employer I have been working for past 10 years for the same employer and none of prior W2s indicate the amount my employer mentioned on my ER. I spoke to my HR and they assured me that it should not be a problem during interview. I just want to get some expert opinions here... your feedback is appreciated.

As long as the XX,XXX is equal to the salary mentioned in your LC, you are
fine. Your current salary which is 10K less does not matter.

Does this employment letter look fine?

I got this format for notarized employment letter for Consular processing. Could anyone went through this process recently check it up for me to see if it is ok? Or I should get a more specific letter specifying the position, description of responsibility and pay instead? My interview is Dec. 27. I don't want to screw up the interview. My case is AC I 140. Thanks a lot for your experience and opinion!
