Questions regarding I134 affidavit form

I am trying to send the I134 affidavit document to my parents. Can somebody please clarify few points
1. My husband is on H1-B (He is the sponsorer) . In the form item 1 - Its asking to pick the category regarding status. We are on H1-B and also we are in AOS process. We have already received I485 receipts,EAD cards and AP. Do we need to mention H1B or AOS and where do we mention that
2. My husband doesn't have birth certificate.Do we need to send his or mine?

I would really appreciate if you can answer my questions.

A2: What is the need for the BC? One does not need to be related in order to provide I-134. Ideally the visa applicants should never need an I-134.
