Questions regarding DS156

hi,

I am going to vancouver canada on 31st October for H1 stamping(F1- H1).

I have couple of more Questions regarding DS156

a) Question 15(Home address) Should this be India Address or the my currect address in US

b)Q 31(Have you ever been refused US visa) I got my F1 on second attempt, So should I answer "Yes" for this Question?

c)Do we have to fill DS 158 form


I would appreciate anyone answering these Questions

a, current address in US is fine
b, answer yes and describe what and when, if space permits quote approval date too.
