Questions on new H4

I am currently working on H1B valid till 2009. I am getting married in December and planning on bringing her on H4. My H1B is already stamped.

Questions:

1. Should her passport have my name on it as her husband?

2. Her current passport has her father's name as last name. Should I change her last name to my name (with regard to H4 visa)? Is it necessary? i have booked the visa appointment using her father's last name. i am not sure if changing her name now will cause any problems.

3. Has anyone gone for H4 stamping in Chennai recently? What sort of documents and questions do they ask?

-------------better it should have. Mumbai US consulate needs that. Or she should get new passport with married name

--------------when she gets her new passport copy they will cancell the old passport and at the time of Interview she should take both the passport stapled
