1.On Form 1-131 instruction, page 5 it says under initial evidence:
If you are an applicant for adjustment of status, a copy of the USCIS or former INS receipt as evidence that you filed the adjustment application;
My understanding is that you can file I-131 concurrently with I-485. If so, then I wouldn’t have the receipt that I filed. Is this requirement satisfied by filing them concurrently?
2.It also asks for passport style photos. As far as I know, this form is the only one that specifies the old photo standard which is 3 quarters of your face with your right ear. The form was updated in 05. Is it safe to assume it is just outdated and that all forms should be submitted with photos by the new standard?
3.Sorry if this is obvious...On the form under Part 2, it asks to choose the application type. I am filing 131 with 130, 485 and 765 concurrently. Should I check “d” which is “I am applying for an advance parole document to allow me to return to the US after temporary foreign travel. " I thought maybe I could be under "a".

1.Just wanted to check… it only asks for 1 photo of each – husband and wife. I just thought it was unsual since all other forms ask for 2 duplicate photos. Is this correct?
2.Many forms ask for the copy of I-94. On the form 130, it asks for the number and the entry date. I entered US last time using a H1 Visa which expired last year. I have the new VISA approval notice with new I-94 at the bottom but I haven’t gotten the actual VISA on my stamp yet as I haven’t left the country since. So I have two I-94 cards and I don’t know which one they are asking.

3.Medical exam report.
It is unclear when you are supposed to and who is required to submit the report. It says ‘if required…” in one line and ‘if you are filing…include medical exam report with the application’ in another line. Which is true? Is it a good idea to just get the exam done and submit with the application?

Medical examination.
When required, submit a medical examination report on the form you have obtained from USCIS.
General: If you are filing your adjustment of status application with a USCIS service center, include your medical examination report with the application, unless you are a refugee.
