Questions in DS-156 and DS-157 form

Hi all,
I am going to Ciudad Juarez, Mexico this month 25th for changing my VISA from F1 to H1B. I have F1 visa till 2005. I got my M. S. degree and am working for almost 2 years now. Got all paystubs so far with all tax returns and W-2's. I have few quuestions on DS-156 and DS-157 form:

Q1) On DS-157 form, what should we write for question3 "Full Name(In Native Alphabet)??"

Q2) On DS-157 form, what should I write for question18 "Have you made specific travel arrangements?? If YES, please provide complete itinerary for your travel, including arrival/departure dates, flight information, specific location you will visit, and a point of contact at each location.??"

Q3)On DS-156 form, what should we write for question26 "How long do you intend to stay in the U.S.?"

Q4) On DS-156 form, what should we write for question28 "Who will pay for your trip?"
