Questions - H1b - revalidation ?

Hi,

I am H1b holder and planning to revalidate from India, so I started filling the Ds-156 and
DS-157 form.

I have some questions regarding the DS-156 and DS -157 form.

DS -156 :-
----------

1. How long do you intend to stay in the US ?
2. What is the purpose of your trip ?
3. Have you ever been issued a US visa ?
(This question confuses me, since i got my first stamping from chennai and last one from US.so what should give for WHEN and WHERE question ? )

4. Name and relationships of persons travelling with you.
( My wife may join with me , while iam going back to US . so what should i answer here, since i am not sure about this )


DS1-157 :-

One question is there , to "mention my two previous employers details" :-
- my previous employer has closed his company and there phone number does not exists. so what should i do for that ?

Another question is , "have you made special travel arrangements ? if yes, specify the location
you visit, and a point of contact at each location"

- I am working there , what specific location and a point of contact , I have to give in this column ?

Please advice me !

--- You are not going for REVALIDATION you are going for H1 visa stamp, the term revalidation is used when you get visa revalidated by DOS in USA

--- Write day expiry date on your latest H1b approval notice
2. What is the purpose of your trip?
Nonimmigrant worker, (H1B1 status)
3. Have you ever been issued a US visa?
(This question confuses me, since i got my first stamping from Chennai and last one from US.so what should give for WHEN and WHERE question ? )
--- Please see the attached SHEET A(attach sheet A with form ). On sheet A write that you got first time H1 visa stamp from US consulate Chennai, India on date/year and second time H1b visa stamp was revalidated in US by DEPARTMENT OF STATE on XYZ date

( My wife may join with me , while iam going back to US . so what should i answer here, since i am not sure about this )
---- If you are not sure nobody is sure, if she plan to come with you then write the Name and relationship
