Questions about IRS FORM 4506-T

I am trying to fill this IRS form 4506-T. This is in case I get a RFE I must
have transcripts of my tax return handy. Has anybody filled this form?
Maybe you can help....

Item 6 is kind of confusing.
What should I enter in
Enter the return number here and check the box below >_______________

What should I be checking?
a) Return transcript
b) Account Transcript
c) Record of Account
d) Verification of Nonfiling
e) Form W-2, Form 1099, Form q098, Form 5498 series transcript

If I want the tax returns for last 3 years should item 7 be

01/01/2001 01/01/2002 01/01/2003

What should the dates be ? I would think just the year would be enough.

Can somebody please explain? People must have filled this up when they
got their RFE. No?

When I filled mine out last week, I did the following:

- return number = 1040 (i.e. the type of tax return you are requesting)
- you want a return transcript (unless you've lost your W2s, then select that option too)
- my guess regarding years was to put the year end, not beginning: i.e., 12/31/2001, etc. Your return is for the period from the beginning of the year to the end, so you are seeking returns for the 'year(s) ending..."

I can't verify this is exactly correct, but it's how I interpreted the form.

Call them instead

Call the number on the site and follow the prompts. There are options to get the transcripts from 1996 onwards. Its easier than filling the form.
