Questions about H4 extension. Jim, Jaxen or somebody else, please help!!

Hi,
I am planning to apply H4 extension to my wife. I figured that I have to file form I-539. I haven\'t contacted INS directly before. If somebody can relpy to my questions below that would be great.
1)Do I need to send a personal check or bank check? (I think the fee is $140)
2)Do I need to send it by priority mail or fed ex? Also, do I need to enclose any self-addressed envelope?
3)What about the cover letter? Can I apply on my wife\'s behalf or she should address it directly and sign the cover letter?
4)What documents I need to attach? ( I can think of H1B approval, marriage certificate, her passport copies..)
5) Any ideas about how long it takes for approval?
