Questions about extension of stay

I am planning to extend the stay for my parents, I have one question, Should I pay the fees seperately for both applicants or only for the primary.
I tried to do e-file and there it asks the fees only for Primary.
 
We have booked the tickets for 6 months and they got only 3 months I-94 and we are planning to take them to different places during November and December and 3 months stay is not enough to stay with us....

Pay fees for both of them. What is the reason for the extension of stay?

They got 3 months due to I-94W (green in colour) or is it a restricted I-94 (white in colour)? If it is the latter, then I advise you not to file an I-539. A restricted I-94 means that the officer at the POE already had doubts about the intentions of your parents. Filing an I-539 will only feed the suspicion.

i need advice whether i should apply for an extension or not.

here's the scenario:

have B1/B2 multiple visa...

its my first visit here in US and i was given 6months stay. it shows in my I-94 that i am eligible to stay here until FEb 08/08.

however, my fiance is giving birth between feb11-MAR3 of 2008. i would like to be with her during this dates but my stay is no longer valid unless i would request for an extension.

Qs:

1. is it better to go back to my country in DEC (last week) and return FEB (before my fiance giving birth) instead of requesting for an extension?
2. or... is it better to request for an extension rather than going back to the US after just 1-2 months of stay in my country?
3. what are the documents needed so that there is a high possibility that the extension will be granted?

please help me out coz i dont know what to do and i dont want my visa (multiple) get affected..
