Hi,
I have the following questions in form DS11.
- There's a section to state 'permanent address'. I am currently renting, and likely change address in future. What would be considered permanent address in such cases ? Address in country of origin ??
- It asks for a travel date. I will likely travel sometime this year, but dont have a concrete travel plan. Does this mean that one can only apply for US passport if they have a concrete travel plan ?

Put the address of your current place of residence, if you think you will stay there for at least 4-6 weeks. That is how long it takes to receive your passport from the Dept of State. They do not care about your country of origin, so they need your US address even if it is not permanent according to you.
