Questions about assembling our I-130/ I-485 packets

These questions might seem silly but this is very essential thing for me and I don't want to mess it up.

Each one of the forms (I-130 & I-485) requires that I send a marriage certificate certified copy and evidence of a bona fide marriage.

My question 1 is, do I have to send the same stuff in both packets? or shall I just put them in one of them and refer in my cover letter that these stuff are included in the other packet? (FYI: I am an F1 student)

Question 2: Do I have to put form G-1145 in the first page of each packet?

Question 3: In form G-1145 do I put the applicants name or the petitioner's name or do I put both of them (my name and my wife's)?

Question 4: which one comes on top of the application G-1145 or the money order?

IF you qualify to file concurrently, everything goes in ONE application package. Only an immediate relative of a USC who made a lawful entry--even if you overstayed (probably excluding a Visa Waiver overstay) can file the USC's I-130 and the alien's I-485, I-765 and (if eligible I-131) all together with ONE set of evidence. Concurrently filed I-130 and I-485 stay together in an A-file while the other applications for interim benefits are separated by the USCIS Lockbox folks. The G-1145s should be attached to each form to ensure proper e-notice and/or texts for each form.
