Questions about AR11

My I-485 RD is 02/28/2002 and ND is 03/22/2002.
I am moving locally next week. I have these (dumb) questions about AR11. Could any of you clarify these please?

1) "I AM IN THE UNITED STATES AS:"
Should I check "Other:" (and write I-485 Adjustment of Status) ?

2) "COPY NUMBER FROM ALIEN CARD"
Is this the A number from the I-485 receipt?

3) "PORT OF ENTRY INTO U.S. and DATE OF ENTRY INTO U.S."
Is this my latest Entry or the First Entry

4) I"F NOT A PERMANENT RESIDENT, MY STAY EXPIRES ON: "
My H1B has expired and I am on EAD. What should I enter here? (Adjustment of status?)

I would really appreciate any help on this.
 
I guess you could use following answers (disclaimer: this is not a legal advise, just a personal opinion):

1. Other I-485 AOS
2. This is your 'A' number (file number) from either EAD card or you may find it on 485 receipt notice (I'm not sure, this is certainly mentioned on EAD card).
3. Your last entry as per I-94
4. As per date on your I-94 or if it is an old I-94 then the date on your AP.

Hope this helps, the legal dept of your company would extend the best advise to you. Above answers would be my personal preference.

Thank you very much. Just one more quick question:

My H1B has expired and I don't have Advance Parole Visa. So, for "IF NOT A PERMANENT RESIDENT, MY STAY EXPIRES ON: " can I just write "AOS"? Any other suggestions?

Just put N/A.

For the last question I just put N/A as per my attorneys advise. It seems there is no expiration until I-485 approved. Applicant can stay in US as long as AOS is pending for approval.
