Question regards to DSP-122 form

Hello all,

Please answer the questions that I have.

DSP-122 Form:

Section 6) Education:

Do I have to list all the schools that I attended, not just University, like High School ?

Section 7) Work Experience:

7a) Within the last 5 years, I have worked at least 2 years in a job that requires at least 2 years of training or experience.

7b) Occupation - if you answered "yes" to question 7a), give job title and describe the type of work you did. Be as specific as possible.

7c) Names and addresses of your employers during the past 5 years in the work you described above, and the dates (Beginning and Ending Months and Years) you worked for each.

I have a undergrad degree in engineering from a Canadian university and worked for 9 years as Computer System Analyst. Do I have to answer yes to 7a) and provide 7b and 7c details?

SECTION 6 EDUCATION: If you are qualifying through education (4 years high school)
tick the box that reflects your highest level of education. Then in the list include all the schools, the elementary school(s), high school and also university, use additional sheet if necessary


SECTION 7:
It means you are qualifying through work experience, not through education, it applies to people who did not completed 4 years high school (although if you decide to qualify based on education, section 6, it does not mean that you cannot include work experience too, section 7, they do not exclude each other in any way)

If you are qualifying through work experience:
- answer "yes" to 7a), if the position was held within the last 5 years
then provide a lot of details in 7b and then list the employers who have been giving you the jobs in 7c

Section 6: You only need to list schools attended starting from high school to colleges/universities. Elementary school is not required.

Section 7a: Answer Yes
Sections 7b & 7c: Yes, provide the details

Answering the questions in Section 7 helps provide a more accurate information about your person to the CO. It is not an indication of you qualifying or trying to qualify through work experience.
