question regarding yearly H1B extention

Hi,

I am in a 3-year clinical residency program and the Hospital extends the contracts on a yearly basis, thus necessitating yearly H1B visa extensions.
When I get the apporval notice for each year, I need to go to my home country and get a valid visa (valid for one year only) from the US embassy. When I reenter the US my I-94 indicates a date that correlates to the visa expiration date.
Does this mean that I need to leave the US at that date again and get a new visa from my home country's US embassy, or can I stay beyond the date indicated on the I-94?
If that were the case, it would mean that I would always have to schedule my vacation time close to the end of the expiration of the visa (otherwise I need to apply very early in the year for a new approval notice each year).
If people on H1B visas were to apply for a Green Card, would that not mean that there are travel restrictions? And if there are travel restrictions, would that not cause problems in getting the yearly H1B visa extentions from your home country's US embassy?

You no need to go every year to get your visa stamped in your passport. Whenever you extend your H1B either one year or 3 years you always get a I 94 at the bottom of the H1B approval notice, which gives your status. Visa in the passport only facilitate multiple entry in to US at the Port of Entry.

You can renew your H1 without going out USA and stay in US any number of yeras. Your valid status is determined by I 94 not by the visa stamp in your passport.
