Question regarding supporting documents for N-400

Hello guys,

I am working on compiling the supporting documents for N-400. All I have so far is 1. copy of the Green card 2. copy of Marriage certificate 3. U.S. citizen spouse's passport 4. IRS transcripts for 2008, 2009 & 2010 5. Joint bank statements 6. Joint Utility & cable bills. I have few questions:-

a. Do I need to send photos of my wife and I with the application?
b. I have a traffic accident ticket from 2004, and the proof of payment($93 fine) from DMV database. Do I need to include this info in the application or just take these documents with me during the interview?
c. Is there any other supporting documents I should include for N-400 application based on 3 years of marriage to USC?
d. N-400 Part 1-B asks for name exactly as it appears on the permanent resident card. In my green card, "firstname, Middle Initial, last name" is printed. So, Do I type my middle initial for this question or print my "full middle name" ?

a) No, don't send photos of her.
b) Bring them to the interview, don't send them.
c) What you have seems sufficient.
d) If they put your middle initial on the card, write your middle initial for that part of the question.

a) Only applicant's photo is required with application, not USC spouse.
b) Bring it to interview.
c) Refer to document checklist in guide http://www.uscis.gov/files/article/M-476.pdf
d) Type the name exactly the way it appears on GC
