Question regarding I-94

Currently I am working on EAD and my I-485 was applied in Nov 28 2001 at VSC. My H1 expired and so my I-94 in August 2002.

Question,
Do I have to get a new or apply for renewal of I-94,

if YES, what are the forms and any details.

No your status is OK, you do not need a I94 as long as your 485 is pending.

NOV28 your case is not unique. This type of thing happens with lots of people. I-94 is just a record that you entered in this country legally and from where.
Till you go out of country you can continue to hang on with your current I-94.

I read a Murthy article and got confused. Hope now with 485's getting approved, all the trauma ends.
