Question regarding AR11

I need to fill the AR-11 Form for self and wife. I am on the seventh Year H1 valid thru 09/04. However my wife's H4 application is pending at VSC. what should i write for her status and Validity date.
I am still working on H1 haven't used EAD, wife received EAD card valid thru 10/04. Can i write my status as H1 and wifes status as EAD on the form or should i say H4 pending and give them the Lin Number

if her H4 extension is pending write it in cover letter and attach her copy of H4 receipt

Hi Ginnu
I changed the address with NSC on 10/20 and mailed out AR11's yesterday for both my wife and me. I received a letter from NSC about the change of address for me but haven't received any thing for wife. Is this normal that they send out a letter informing us about change of address.
