Can anybody suggest how to mention part time jobs in this Question, or I don't have to mention part time jobs .

I'd suggest you list ALL the jobs you ever had. Be it part-time, temp or something else. I listed my temp assignments and internships, everything.
It's always better to overdisclose then underdisclose.

Thanks vera979 for a quick reply, I think I should use a seperate sheet for part time jobs because all the time I had full time job last 5 years besides a part time job, so using seperate sheet will not creat the confusion between the dates. Please respond

I believe they provide you with 5-6 lines there to list your places of work.
If you only had two jobs , one full time and one part time i do not see why you should use a separate sheet for part -time jobs. You would use a separate sheet if you had too many to list and you ran out of space.
Again I'd list current or the latest job first tnd then your part time job on the line under it.
