Question on PIO Card

First of all I apologize to post this in a wrong place.

I have a simple question.
When applying for PIO card for my 5yrs old Son, Do I need to sent his US Passport?
The Application form is simply asking to sent the Photocopy of the Passport.
Not sure if that means Passport+Photo copy(Is it implied to sent the passport)

Please let me know if somebody knows the answer.

This helped me a lot. I was about to sent the Passport then I talked with my co worker,He told me that PP is not needed but just Notorized PP Copy.
With you also confirming I am good to go

I am also applying for PIO card for my kids. What supporting doucments are you sending? Accoridng to my knowledge we have to send notarized copy of our passports and kids passports. Is that sufficient?

This is what I sent

1. Application Form [in duplicate].
2. Four passport size photos.
3. U.S.Passport notarized copy.
4. Copy of Birth Certificate for the applicant.
5. Notarized copies of Indian Passports of both the parents.
6. Proof of Permanent Address & residence in India [copy of address in father’s passport & copy of school & college certificates, copy of insurance policy, copy of birth certificate for father (as per questions 12 & 13)].
7. Proof of current address [copy of bank statement & utility bill].
8. Cashier’s Check in favor of the consulate general [$155 + $15(overnight mail)].

I am also planning to appy for the pio card for my son he does not have a Passport can i send the Birth Certificate will it suffice.

I emailed to the consulate they replied on my Foll Questions
hope it helps anyone with similar questions

a) Under the guidelines (No.5) will birth certificate alone suffice ?

b)What documents can I submit for his proof of residence ,since he is an
infant can we parents submit our proof of residence?

parents passport fine

c) Do visa camps accept P.I.O applications ?

no
 
I also got reply from Indian consualte, the follwing list of documents should be sufficeint

2 set of completed application forms for each application
4 passport size photographs
Notarized copy of my wife and my Indian passport
Notarized copy of kids US passport
Money order of $170 (155+15)

If you are sending two applications then send $15 for both applications.

Kulfi, your list looks very complete!

Yasmine, its good to confirm that copy of parent's passport can be submitted for proof of permanent address in India.

1.) In application form, Q.14. Do we have to fill that with parent's details or we can leave it.
2.) Do we need to notorize application form also
3.) For question where we have to name two persons living in india known to the application. Do we have to give any proof about that. I will be mentioning applicants grandparents names. is that ok.

No. Only Passport copies of Parents and Applicant

No. Name and address is sufficient.

Got card/booklet around 3/31/05. So im my case they had turnaround time of 4 weeks, not bad!
