Question on PIO Application Form for minor

With 79 views and not a single response in OCI thread, I thought of posting this message here as well to get some help.


I am filling PIO Application form for my 5 year old son and have the following questions and would really appreciate your suggestions:

1. Should I paste Passport Size Photo on both the sets (DUPLICATE form also)?.

2. What should I do for "Please sign in the box below" for a minor?

3. What should I write for Sec 13. "Date of Expiry" for our passport details as we got our Indian passports canceled after Acquiring US Citizenship & I don't remember the date the Indian Passport was canceled.

Don't take this the wrong way but if you read the sticky posts (The ones that are always on top) and read the forums for all other messages, you will see these questions were answered few times before. Still..

1. Yes, separate photo on each for form.

2. As instructions says, for minor, you put a thumbprint

3. I would put date of expiry as shown in the passport. Passport is used as basic to determin Indian origin. So even if it is cancelled before expiry, it was still Indian passport.

For (3), Some people suggested that I should put in the oath date.
