Question on I130 Line 22

I am here in US on H1B and myself and my wife are living together.Filling of I130 Line 22 says

"Complete the information below if your relative is in the United States and will apply for adjustment of status"

Do we need to specify here Chicago Lockbox ( where we are sending forms to) or currentl city where we are located..

QUestion on I130 Line 14

As part of my H1 visa renewal i got my new I94 and expiration date which comes with I797..Do I need to specify that I94 under Line 14 or one on my passport ?

Why are you filling I 130?
 
Assuming that your wife is a USC and that she is filing an I-130 on hyour behalf CONCURRENTLY with your I-485 etc.., it is all filed together and will stay together and will end up in your A-file at the local office where you will be interviewed. To find out whch office that is, go to www.uscis.gov and locate your local field office by entering your zip code....
