Question on DS 156

Hello,

I am working in USA and am going for visa stamping at Hyderabad. Have couple of questions on DS 156 form

1.Question No 15 (Home Address) - I believe this is the address in India. Is that correct? or should i specify the USA address

3. Question No 28 - Who will pay for the trip? Would SELF be a correct answer?

4. Questino No 29 - Have you ever been in USA and how long ?
I have been here for the past 8 years with vacations in between. What do we give here shoud we say 8 years and does this need to be accurate with specific months and days ?

Enter Additional visits to US - should we list all the visits in the last 8 years

5. Question No 30- Enter additional visu issuances here ? (I will be listing all H1 and F1 visa's . I believe this is only for us visas, is that correct? or should I list the visas issues by other countries as well)

A1: US address
A3: Answer "self" is correct
A4: 8 years is fine. If you can state the months, that would be better.
A5: US visas only
