Question about supplemental to 693

The supplemental to 693 form has bunch of columns for filling in dates for transferred written immunization records. Then there is a column for the dates when the Civil Surgeon administered the vaccine (if any) and then another column where is supposed to check if you are immune.

The question is, if the column that indicates you are immune is checked, and you also have a medical record which shows that you got some vaccines in a different place somewhere in the past (not related to the GC medical exam), is it required to fill in the dates in the 693 supplemental, or it is enough to attach the document to the RFE response?
I wish I could ask the doctor, but he left on vacation today!
 
Dates are required if the Dr. filling out the application actually administers a vaccine. Otherwise they typically end up checking the "not appropriate age" column. At least that's what they did on my form. At the bottom they will check the box stating all vaccine requirements are met etc.

I also have a medical record that shows some vaccines that my dependent had done last year (not related to GC app). Is it a good idea to include these records along with the RFE response and the sealed envelope from the doctor?
