Question about RFE ...

Has anybody ever have had a situation before where the INS online status shows that an RFE has been issued but the RFE actually never arrived by mail to the lawyer.

I am somehow not very convinced when people say that it has got lost in the mail.

Also, can we call INS and have them request them to re-issue the "lost" RFE ?

If I were you, I would be very concerned... All RFE's comes with a 90 day deadline for receipt of responses by USCIS... If the attorney has not received the RFE, even after 14 days... one needs to contact USCIS to have it re-issued. Thats what I would do if I were in your position.
 
What I'm more concern is kind of the opposite:

Has anybody had a case where RFE was received by the attorney BUT not reflected on the online message?

If 14 days have passed and you have not received it, I think the onus is on you to inform USCIS that you have not received the RFE. I would do it in writing too.
 
U can call INS

On the national toll free and they will reissue. then u must montoir the online status to make sure that they resend- as it will be reflected in the message with a new Rfe sent date
happend to me for 140 and also ead- just plain bad luck. hurts when u are waiting for years and then have to wait endlessly
